Early Malnutrition's Hidden Impact on Insulin Function

The SAMPA study gathered data from children and adults across Zambia, Tanzania, India and the Philippines. Researchers compared those who had experienced prior wasting malnutrition (PM) with those who never had it (NPM). In total, 2,172 participants were analyzed. The goal was to see how PM relates to insulin sensitivity and secretion, and whether current body weight changes the picture.

Kids who had PM tended to weigh less today. Their insulin sensitivity, measured by the Matsuda index, was 20 to 24 percent lower than kids without PM. This suggests that early malnutrition can blunt the body’s ability to respond to sugar.

Adults showed two different patterns. Those whose PM happened decades earlier (about 36‑37 years before the test) often carried extra weight now. Even after accounting for their current BMI, their Matsuda index was 18 percent lower, indicating reduced sensitivity. In contrast, adults whose PM was more recent (3‑17 years ago) mostly stayed underweight. They produced less insulin (HOMA2 %B was 10‑20 percent lower) but their sensitivity was higher (Matsuda index 21‑47 percent higher) because they were lighter.

Overall, the body tried to balance things. When sensitivity dropped, insulin secretion rose, keeping the disposition index and the rates of diabetes and pre‑diabetes similar between PM and NPM groups. The study shows that the effects of early wasting depend heavily on whether a person stays thin or gains weight later. This insight helps explain why diabetes develops differently in low‑and middle‑income regions.
