Fentanyl Purity Drop Didn’t Fully Explain Overdose Decline
Overdose deaths hit a record high during the unregulated fentanyl era. Recently, those numbers have started to fall. One idea suggested that a sudden drop in fentanyl purity beginning in 2023 caused the downturn. Researchers wanted to test this idea. They repeated a published analysis using regional overdose data. Their method included transfer function models that accounted for time trends and autocorrelation. Negative controls were added to guard against false connections.
The findings showed that rising national fentanyl purity did not line up with fatal overdose rates in most U.S. regions. Only a modest link appeared in the West. When both purity and deaths later declined, similar patterns emerged with unrelated economic indicators. This raised doubts about whether the original link was real. Canadian subprovince studies also reported mixed strength and direction of the relationship between purity and deaths.
Law enforcement purity measurements may be biased. In 2023‑4, higher‑purity wholesale samples were more likely to be reported. This selective reporting can skew the data over time. Such bias limits how well national purity figures reflect actual regional changes. Because of this, the data may not be fully reliable for longitudinal or geographic analysis.
National fentanyl purity alone cannot explain the recent decline in overdose deaths. Local drops in purity might help explain some regional drops. However, many other factors likely played a role. These include changes in drug supply, public health programs, demographic shifts, altered user behavior, post‑pandemic recovery, cartel dynamics, and fewer people starting illicit opioid use. Researchers say these alternative causes need further investigation.
