Hip Pain Codes and Physical Therapy Benchmarks for 2025

United StatesMon Nov 02 2026

Many older adults deal with hip pain. In the United States, about 19.6 percent of people over 55 have signs of hip osteoarthritis on an X-ray. Outpatient physical therapists treat this condition all the time. It is one of the most common issues they see in their clinics.

The American Physical Therapy Association wants insurance companies to take physical therapy seriously. They created new benchmarks for 2025. These benchmarks help payers understand how much treatment a person might need. The goal is to show that therapy is valuable and worth covering. Payers need clear data before they approve a course of care.

Insurance claims use special codes. For hip osteoarthritis, there are several options. M16.1 covers primary arthritis in one hip. M16.0 covers both hips. Other codes handle dysplasia, injury-related cases, or secondary causes. There are also separate codes for hip stiffness and hip pain. Picking the right code matters for getting approval.

This system affects real people. When a therapist documents care, the code tells the payer what is happening. If the code does not match the situation, payment might be denied. Patients should ask questions about how their condition is classified. Understanding these codes can help people advocate for the care they need.
