The breathing muscle on one side of the chest can be slowed after a shoulder nerve block.
Researchers tested whether using smaller amounts of medicine could reduce this side effect without hurting the block’s effectiveness.
They gave three groups of patients different volumes of local anaesthetic during an interscalene brachial plexus block.
Each group received a unique dose, while all other factors stayed the same.
After the block, doctors checked how well the diaphragm worked on each side and whether the patient could lift their arm comfortably.
The results showed that when less drug was used, fewer patients had a weakened diaphragm on the side of the block.
However, the success rate of the nerve block stayed high across all doses.
This means that a lower volume can still provide good pain relief for shoulder surgery while sparing the breathing muscle.
The study highlights that careful dosing of local anaesthetics can protect lung function without compromising surgical comfort.
The findings encourage anesthesiologists to rethink standard volumes and tailor them to each patient’s needs.
