Some Ohio lawmakers aren’t convinced Cleveland needs a third Level 1 trauma center, despite plans by Cleveland Clinic to build one by 2028. Eleven state legislators recently asked the hospital system to pause and prove why the region truly needs the extra facility. They argue that expanding care should come only after checking if patients would benefit—or if it’s mostly a business move.
The debate isn’t just about numbers. Critics worry adding another top trauma center could disrupt existing care rather than improve it. MetroHealth, which runs one of the two current Level 1 centers, says rushing ahead without a clear study could strain staff and resources. Meanwhile, Cleveland Clinic insists it needs the new center to keep seriously injured patients from being transferred elsewhere, though it hasn’t shared full regional data to back up the claim.
Money and influence also play a role. The Clinic says donations, including a $30 million gift, will cover costs—but lawmakers note that donor interest doesn’t equal medical need. Staffing is another issue. Ohio’s trauma doctor shortage means hospitals compete for the same experts. If Cleveland Clinic pulls talent away from others, could there be unintended consequences for patients?
The bigger picture raises questions about how trauma centers get approved. National guidelines suggest one or two high-level centers per million people, but real-world decisions are often driven by hospital goals, not just patient demand. Cleveland has seen its fair share of trauma center closures over the years, including MetroHealth’s original predecessor in 2000 and Huron Hospital in 2011. Each time, the system reshuffled—but did it get better?
No one disagrees that severely injured patients deserve fast, expert care. The question is whether piling on another Level 1 center is the best way to make that happen—or if it’s just another hospital in a crowded field.
