Older people with heart failure are increasingly turning to digital tools like apps, wearables, and online programs for help. A recent review looked at how well these tools are tested in real life, especially focusing on who actually participates in the studies and whether everyone gets a fair chance to benefit. The researchers used a framework called PROGRESS-Plus, which checks important social factors such as age, gender, race, income, and education.
The first big finding is that the studies rarely describe participants in detail. Many trials only report basic age and gender, leaving out crucial information about ethnicity, socioeconomic status, or living situation. Without this data, it’s hard to know if the results apply to all older adults or just a narrow group.
Second, engagement levels vary widely. Some studies show that participants regularly use the digital tools and report improvements in daily life, while others see low usage or drop‑outs. The review highlighted that factors like digital literacy and support from caregivers play a big role in keeping people active. When older adults receive help to learn the technology, they are more likely to stick with it.
Third, equity is a major concern. The review found that many trials do not address how to make digital health fair for everyone. For example, there is little evidence that tools are adapted for people with limited internet access or low health literacy. The authors argue that future research should explicitly plan how to reach underserved groups and measure outcomes across different social backgrounds.
The overall message is that while digital health offers promise for older adults with heart failure, the evidence base needs to be more inclusive and transparent. Researchers should report full participant details, design studies that support sustained use, and actively work to eliminate digital divides. Only then can we be confident that these innovations truly help all older hearts, not just a select few.
