Continuous Glucose Monitoring (CGM) devices can help people manage their diabetes with greater precision and ease. The Libre Pro device is a significant advancement in this direction. It provides real-time glucose level updates, making it easier to manage the disease. The goal is to reduce the gap in health outcomes among communities and improve the lives of those who can't access the best care.
In a recent project, CUHCC introduced the Libre Pro device to patients without insurance. The program aimed to make diabetes management easier and more reliable. The clinic used a multi-disciplinary team of healthcare professionals, including pharmacists, nurses, and clinicians. They worked together to provide support and monitor progress.
The team used the HbA1c test to measure the effectiveness of the treatment, identifying diabetic patients with a mean HbA1c level of 9. 73%. These patients were diverse in age, race, ethnicity, and language preference. Over 35% of them spoke Spanish and over 14% were Somali. They were also diverse in their clinical characteristics with an average BMI of 29. 91 kg/m2. The key factors impacting diabetes patients is race and ethnicity.
