Understanding mental health struggles among sex workers in Tanzania

Tanzania’s sex workers face serious mental health challenges that often go ignored. Studies show nearly half of them deal with depression, while over 40% struggle with anxiety and one in five shows signs of post-traumatic stress. Shockingly, a third have thought about suicide, and nearly 8% have actually tried it. These numbers reveal a hidden crisis where violence and risky conditions push many to the edge. Violence plays a huge role in worsening mental health. Women who faced gang rape were far more likely to suffer from depression, anxiety, or PTSD. Even everyday abuse, like attacks by strangers, links closely to depression and suicidal thoughts. Early sexual trauma also leaves deep scars, increasing the risk of PTSD and suicide attempts later in life. These patterns prove that trauma isn’t just personal—it’s shaped by dangerous environments.Some struggles are tied to daily life in sex work. Working with many clients in a week raises anxiety levels. Raising three or more children alone makes PTSD more likely. Moving often for work adds to stress, while living in certain areas or having extra income sources can push some toward self-harm. These connections show how poverty and unstable living conditions feed mental health struggles in ways many outside the industry might not realize. Surprisingly, alcohol use doesn’t just happen on its own. It often becomes a way to cope with abuse, making anxiety and PTSD worse. While moving for work doesn’t change these patterns, the drinking habit deepens the damage. This suggests that breaking the cycle requires more than warnings about alcohol—it needs real support to escape violent situations first. Experts argue that mental health care for sex workers must do more than hand out pills. Programs should focus on stopping violence, providing safe housing, and creating stable incomes. Without addressing the root causes, treating depression or PTSD alone won’t be enough to help these women heal.
