What 10 VBHC Hospitals Learned While Changing Care

Hospitals are trying to rebuild care around whole conditions. This sits inside value-based health care, often called VBHC. Some places are using integrated practice units. Others are testing condition-based redesign. The idea has real appeal. Still, the practical steps are not always clear.

That practical gap is why the Value Catalyst effort stands out. It brings together 10 hospitals from around the world. These hospitals are known for advanced VBHC work. In learning meetings, each hospital explained how it was moving toward a condition-based organization, or CBO. The group used those lessons to shape and revise a working framework again and again.

After the meetings, the hospitals answered a questionnaire. The survey asked how each hospital was putting the framework features to work inside its own organization. That step helped turn meeting insights into a clearer map. It also raises a useful question for any hospital: can the team see who owns each step, and can the plan explain how change will happen?

The final framework has three main parts. First, hospitals design the CBO structure. Second, they define roles and processes inside condition-based units. Third, they build a change strategy for the shift toward VBHC. The model also shares good-practice examples from the Value Catalyst hospitals. Teams can then look at whether the structure, roles, and change plan fit together.
